Chiropractic Neurology Center (CNC) operates neurochiro.com. This policy describes information submitted through our website, website services, and how to contact us with privacy questions.
Information you provide
General inquiries. Our contact form requests your name, email address, optional phone number, and message. The form sends your inquiry to the office so we can respond. Please do not include medical records, diagnoses, or other private health information in the general contact form or ordinary email.
Employment inquiries. Our careers form requests your name, email address, position of interest, optional phone number, optional introduction, and optional resume. Applications are emailed to the office for recruitment review and follow-up. Do not include Social Security numbers, banking information, or unnecessary confidential information in an application.
Testimonials. Our testimonial form requests a display name, optional city, your experience, and authorization to publish your submission. Submissions are reviewed before publication. Approved testimonials may appear on the website and in marketing materials according to the authorization you provide. Contact us to ask about a submission or request removal from materials we control.
Patient forms. Our patient intake and progress forms may request identifying, contact, and health information for clinical and administrative purposes. Use the designated patient forms when directed by the office. This website policy is not a HIPAA Notice of Privacy Practices and does not replace the notices or obligations that apply to patient health information. Contact the office with questions about patient records and privacy practices.
Website operation and information handling
Website submissions are processed through the clinic’s website, communications, and form-processing systems. Clinic personnel and service providers supporting hosting, email delivery, storage, and technical operation may handle information to perform those functions. Bluehost hosts our WordPress website. Approved testimonials are also disclosed publicly as described above.
When your browser connects to a website or embedded service, the receiving server receives technical connection information, including an IP address. Hosting and technical services may record requested pages, browser information, timestamps, and errors for operation, troubleshooting, and security.

We are preparing a Google Analytics installation. The new Google Analytics installation has not been activated as of this policy’s effective date. We will update this policy to describe its operation and available choices before activating it.

Browser privacy signals. Do Not Track or Global Privacy Control settings do not, by themselves, prevent your browser from making requests to website hosting or embedded services. This policy does not promise that those services stop collecting technical information when a signal is enabled. Retention and your requests
CNC has not established a single fixed retention period for website inquiries and testimonials. Contact us to ask whether we hold a particular submission or to request its correction or deletion. Copies may also exist in email, storage, and backup systems. Patient and employment records may be subject to separate recordkeeping requirements.
You may request information about a website submission, ask for a correction, or request deletion by contacting the office. We may need to verify your identity and clarify the request before responding. A request does not guarantee deletion of information that must be retained for applicable legal or recordkeeping purposes, and removing information from systems we control does not remove copies already held independently by others.
Please have a parent or guardian contact the office for questions involving a child’s information. Do not send a child’s medical information through the general website contact form.
